Impossible to open voxel file

Hallo every body
I want to use Fluka and flair code in radiotherapy
when i add any dicom file and converted this file to voxel by Flair this message appear with me and the run stop
I attached my input, flair ,err,and log files
can any one help me to solve this problem

can any one help me to solve this problem ?
and why this message appear in all dicom files?

the first messages says that the it cannot find in the current folder the “s.vxl” file.
Can you please ensure that the voxel file is present in the same folder where your input is located and you have read permissions?

my input file has name and the voxle has another name but the voxel file in the same folder
